Stefanie Toadvine, LCSW
Owner/Founder/Clinical Director
Stefanie completed her Master's of Social Work degree from the University of Louisville and soon after discovered her passion working with children and teenagers to help improve their mental health. Stefanie is married and has three children. Aside from therapy, Stefanie also specializes in forensic services and working closely with providing services to family court.

Shelby Fields
Shelby is the smiling face that client's will see as they enter Evolution Counseling. She is in charge of ensuring all clients are scheduled to best meet their needs.
DAWN OAKS
Therapist/Certified Social Worker
Dawn completed her Master’s in Social Work degree from the University of Kentucky. After having raised her own children, she now enjoys working with children and youth at risk. An area of specialization for Dawn is work with women who have a history of depression, anxiety, and domestic abuse. She also has a special passion for supporting parents of youth and young adults who identify with the LGBTQ+ community.
Kelly Falkowski
Targeted Case Manager
My name is Kelly Jo Falkowski. I am from South Eastern Kentucky. I am married and have a sweet and sassy daughter. I have been doing case management in the mental/behavioral health field as well as the educational system for over 17 years. I also serve on the outreach team at my church. My passion is serving others no matter the need. The Lord calls us to serve one another and that is exactly what I want my legacy to be. I am excited and grateful for the opportunity to team up with the amazing people at Evolution and look forward to continuing to serve my community.
Christiana Hubbuch
Targeted Case Manager
Christiana graduated from Eastern Kentucky University and Somerset Community College with a dual degree. She has a Bachelor’s Degree in Science of Public Health and an Associates Degree in Art. After graduation Christiana began working alongside multiple agencies to assist children and families with services and resources. She has a passion for children and loves each family she serves just like her own. She has been married for 13 years and has 8 children. Her passions and hobbies include her love for her family, riding dirt bikes, cooking & also runs her own business on the side of providing case management services.
Jill Kinnaird
Targeted Case Manager
Jill graduated from Eastern Ky University with a degree in Psychology with an emphasis in education. Jill has extensive experience in working with teams, schools and all facets of service systems. Jill has developed a passion for supporting families and children in need. Jill is dedicated to creating positive change for families seeking support. She has 29 years of experience helping others.
